How can I use the template?

The template can be:

• completed as an addendum where an FCA or SIL report has already been submitted.

Where used as an addendum, the template is designed to draw on and summarise evidence already documented within the original assessment rather than requiring you to repeat the full FCA.

This resource is designed for Occupational Therapists working with NDIS participants who receive funded disability supports across the full 24-hour period (24/7) and who meet, or are expected to meet, the High Support Needs safeguard criteria.

It is not intended as a replacement for a comprehensive functional assessment or the OT’s clinical reasoning.
