OT NDIS Templates
Service Agreement NDIS Template
Service agreements outline the terms of service between you as an OT and your client. This foundational NDIS template includes the responsibilities of both parties, payment terms, support details, complaint processes and termination procedures. It's customisable, so you can add your own branding and adjust specifics, such as travel charges or service duration, to fit your practice.
Functional Assessment Templates
Functional assessments are critical for OTs in the NDIS space – but the NDIS doesn’t provide a standardised template. Our functional assessment template helps you structure your assessments to cover all the details the NDIS requires, from assessment tools to recommendations. We offer templates for all functional assessments, including SDA, SIL and ILO assessments.
