An editable OT Recommendation Framework designed to help you document the additional evidence relevant to the NDIS High Support Needs safeguard.

This is not a full FCA or SIL Functional Capacity Assessment report template. It is designed to be added to the report template you already use, or completed as an addendum where an FCA or SIL Functional Capacity Assessment has already been submitted.

The framework includes structured prompts to help you document:

• How the participant meets the High Support Needs safeguard eligibility criteria
• Their existing 24-hour funded NDIS disability support arrangements
• The support ratios required across the day and night
• The level of funding affected by the funding reduction
• How the reduction impacts the participant’s existing 24-hour support arrangements
• Why additional assistance with daily life funding may be required to address the resulting support gap
• The foreseeable functional, safety and support risks if the gap is not addressed

The framework helps you bring this information together clearly without duplicating the comprehensive functional assessment, evidence and clinical reasoning that should already sit within your main FCA or SIL Functional Capacity Assessment.

How can I use the template?

The framework is designed to complement your existing FCA or SIL Functional Capacity Assessment template, not replace it. 

There are two ways to use it:

Option A: Current assessment
Incorporate the framework into an FCA or SIL Functional Capacity Assessment you are currently completing. The framework provides the additional structure for documenting evidence specific to the High Support Needs safeguard alongside your usual functional assessment.

Option B: Assessment already submitted
Use the framework to prepare an addendum where the FCA or SIL Functional Capacity Assessment has already been submitted and additional evidence relating to the High Support Needs safeguard is required.

When used as an addendum, it draws on the assessment findings and clinical reasoning already established in your original report. It is not intended to recreate or replace the full functional assessment.

Who is this template for?

This resource is designed for Occupational Therapists working with NDIS participants who receive funded disability supports across the full 24-hour period (24/7) and who meet, or are expected to meet, the High Support Needs safeguard criteria. 

It is specifically designed to help OTs present evidence relevant to this safeguard, including eligibility, the participant’s required support arrangements, the impact of the funding reduction and why additional Home and Living and/or Assistance with Daily Life funding may be required to address the resulting support gap.

You can add your own practice branding, select the option relevant to your participant and tailor the prompts to their individual circumstances and available evidence.

You will not receive a complete FCA or SIL Functional Capacity Assessment report template as part of this purchase. The OT Recommendation Framework is designed to be used alongside your existing assessment and reporting processes.

It provides a structured framework for documenting evidence specifically relevant to the High Support Needs safeguard and must be tailored to each participant’s individual circumstances and available assessment evidence.

Comprehensive functional assessment findings, participant-specific evidence and clinical reasoning should remain within the main FCA or SIL Functional Capacity Assessment.

The framework is intended to support, not replace, the Occupational Therapist’s professional judgement. Occupational Therapists should use it alongside their own assessment processes, policies, procedures and relevant external advice and ensure their documentation reflects current NDIS requirements and professional obligations.
